Evaluation of the transition process

For adolescents with chronic illnesses and/or limitations, the transition from pediatric care to adult medicine is often associated with major challenges. In addition to medical and nursing aspects, psychosocial, organizational and legal elements also play a role. 
To facilitate this transition, case managers are increasingly used. They accompany patients and families on the transition to adult life and establish contact with the relevant medical specialists.
The transition process should be evaluated with the close involvement of the stakeholders involved, namely patients, specialists, case managers and, if necessary, relatives of the young people, particularly with regard to possible digital support for the process.
